Shoulder CT Scan: What It Shows and Why You Might Need One

What Is a Shoulder CT Scan?

A shoulder CT scan uses rotating X-ray technology and computer processing to create detailed cross-sectional images of your shoulder joint and surrounding structures. While an MRI is generally better for evaluating soft tissues like tendons and ligaments, a CT scan excels at showing bony detail — making it the preferred test when your doctor needs to evaluate fractures, dislocations, bone abnormalities, and the relationship of bone fragments after an injury.

CT images can be reconstructed into three-dimensional views, which are particularly useful for surgical planning. This allows your surgeon to see the exact anatomy of a complex fracture from every angle before making an incision.


Why Would Your Doctor Order a Shoulder CT Scan?

Your doctor may recommend a shoulder CT scan for:

  • Evaluating fractures in detail. While X-rays can identify most fractures, CT provides much more detail about the number and position of bone fragments, the degree of displacement, and whether the fracture extends into the joint surface — all of which affect treatment decisions
  • Complex or comminuted fractures (fractures with multiple fragments) of the shoulder blade (scapula), collarbone (clavicle), or upper arm bone (proximal humerus)
  • Shoulder dislocations — CT can show whether bone damage (such as a Hill-Sachs lesion or bony Bankart lesion) occurred during the dislocation and helps assess the risk of recurrence
  • Pre-surgical planning for shoulder replacement, fracture repair, or reconstruction — 3D CT reconstructions give surgeons a detailed roadmap
  • Post-surgical evaluation to check the position of hardware (plates, screws) or prosthetic components
  • Bone tumors, cysts, or infections affecting the shoulder
  • Arthritis evaluation when bony detail is needed, such as assessing bone loss in severe glenohumeral arthritis
  • Chronic shoulder pain or clicking when initial imaging hasn’t provided a clear diagnosis
  • When MRI is not an option — for example, in patients with certain pacemakers or other MRI-incompatible implants

How to Prepare and What to Expect

Preparation. Wear loose, comfortable clothing. Remove jewelry, watches, and any metal objects near your shoulder. If contrast dye is being used, you may be asked to fast for a few hours and to disclose any allergies or kidney problems.

The scan. You’ll lie on a table that slides through the CT scanner (a large, ring-shaped machine). The technologist will position your shoulder and may ask you to hold still in a specific position. You may also be asked to hold your breath briefly. If contrast dye is ordered, it will be injected through a small IV in your arm.

The scan is painless. The scanner makes soft whirring sounds and takes images in seconds. The actual scanning portion takes about 5 to 10 minutes, with the full appointment lasting 20 to 45 minutes including preparation.

After the scan. You can resume normal activities immediately. If contrast was used, drink extra water. A board-certified radiologist will review the images, prepare a detailed report, and send it to your doctor, who will discuss the results and next steps with you.
