Knee CT Scan: When You Need More Than an X-Ray

Knee problems are extremely common, and while X-rays are the standard first step in evaluating knee pain, they don’t always provide enough detail. A knee CT scan offers a more comprehensive view of the bony structures in and around the knee joint, making it especially valuable for complex fractures, surgical planning, and evaluating bone abnormalities.

What Is a Knee CT Scan?

A knee CT scan uses rotating X-ray technology to create detailed cross-sectional and three-dimensional images of the bones, cartilage, and structures that make up your knee joint. The scan provides far more bony detail than a standard X-ray and can reveal fracture patterns, loose bodies within the joint, and subtle bone abnormalities that are invisible on plain films.

While MRI is generally preferred for evaluating soft tissue injuries (such as meniscus or ligament tears), CT is the superior test when detailed bone imaging is the priority — and it is much faster than an MRI, typically taking just minutes.


Why Would Your Doctor Order a Knee CT Scan?

Common reasons include:

  • Complex fractures. CT shows the exact number, position, and displacement of bone fragments in tibial plateau fractures, distal femur fractures, and patella fractures — information that is essential for surgical planning
  • Evaluating fractures that are difficult to see on X-ray, including stress fractures and subtle cortical fractures
  • Pre-surgical planning for total knee replacement, fracture fixation, or osteotomy — 3D reconstructions allow surgeons to visualize the anatomy from every angle
  • Post-surgical evaluation to check the alignment of hardware, prosthetic components, or healing
  • Loose bodies (fragments of bone or cartilage floating within the joint) that may be causing locking or catching
  • Bone tumors, cysts, or infections affecting the knee area
  • Arthritis assessment when detailed bone evaluation is needed, such as measuring joint space, bone loss, or deformity
  • Alignment studies to assess patellar tracking or lower extremity alignment
  • When MRI is not possible due to implants, claustrophobia, or other contraindications

How to Prepare and What to Expect

Remove any knee braces, bandages, or jewelry from the area. Wear comfortable clothing. If contrast dye is ordered, you may need to fast for a few hours and inform your care team about allergies or kidney conditions.

You’ll lie on a table that slides through the CT scanner. The technologist will position your leg and may ask you to remain still in a specific position. The scan is painless and the actual imaging takes only a few minutes, with the full appointment lasting 20 to 45 minutes.

After the scan, you can return to normal activities immediately. A board-certified radiologist will analyze the images and send a report to your doctor. If contrast dye was used, drink extra fluids to help flush it from your system.
