Stomach (gastric) cancer is a serious condition that can be difficult to detect in its early stages because symptoms often overlap with many other digestive problems. When your doctor suspects stomach cancer or needs to evaluate a known diagnosis, a CT scan is one of the most important imaging tools in the diagnostic and staging process.

Can CT Scans Find Stomach Cancer?
Yes — CT scans can detect stomach cancer, though their role is primarily in staging (determining the extent of the disease) rather than initial detection. Here’s how CT contributes to stomach cancer care:
Detecting the tumor. A CT scan of the abdomen with IV contrast can reveal abnormal thickening of the stomach wall, which is one of the hallmark signs of gastric cancer. Masses or irregular areas of enhancement may also be visible. However, early-stage cancers that are confined to the innermost lining of the stomach may be too small or flat to detect on CT.
Staging the disease. This is where CT is most valuable. The scan can show whether the tumor has invaded through the stomach wall into surrounding tissues, whether nearby lymph nodes are enlarged (suggesting cancer spread), whether the cancer has metastasized to the liver, lungs, peritoneum (lining of the abdomen), or other distant organs, and the relationship of the tumor to surrounding blood vessels and structures, which helps surgeons determine whether the cancer is operable.
Monitoring treatment. CT is used to track the response of stomach cancer to chemotherapy, radiation, or other treatments. Serial CT scans show whether the tumor is shrinking, stable, or growing.
Detecting recurrence. After surgical removal of stomach cancer, CT scans are used during follow-up to check for any signs that the cancer has returned.
When Is an Endoscopy Needed Instead of or in Addition to CT?
It’s important to understand that a CT scan cannot replace an upper endoscopy (EGD) for the definitive diagnosis of stomach cancer. During an endoscopy, a flexible camera is passed through the mouth into the stomach, allowing the doctor to directly see the stomach lining, take tissue samples (biopsies) for laboratory analysis, and diagnose the exact type and grade of cancer.
In most cases, both tests are needed: endoscopy for diagnosis and tissue confirmation, and CT for staging and treatment planning. Depending on the situation, your doctor may also order PET/CT, MRI, or endoscopic ultrasound for additional information.
What to Expect During the Scan
You may be asked to fast for a few hours before the scan. You may be given water or an oral effervescent agent to drink just before the scan to distend the stomach, making the stomach wall easier to evaluate. IV contrast dye will be administered through an IV in your arm.
You’ll lie on a table that slides through the scanner. The scan takes about 15 to 30 minutes and is painless. You may feel a brief warm sensation from the contrast injection.
After Your CT Scan
You can eat, drink, and resume normal activities immediately. A board-certified radiologist at IPMC will analyze the images and provide a detailed report to your gastroenterologist, oncologist, or surgeon. Results are typically available within 24 to 48 hours, and your doctor will discuss the findings and next steps in your care.
