What Is a Bladder Ultrasound?
A bladder ultrasound uses sound waves to create real-time images of your bladder — the hollow, muscular organ in your lower pelvis that stores urine. The test can show the bladder’s size, shape, wall thickness, and internal contents. It can also measure post-void residual volume (PVR) — the amount of urine remaining in the bladder after you urinate. A high PVR may indicate incomplete bladder emptying, which can be caused by conditions like enlarged prostate (BPH), neurogenic bladder, or urethral stricture.
Bladder ultrasound may also reveal stones within the bladder, masses or tumors on the bladder wall, trabeculation (thickened wall from chronic obstruction), and diverticula (outpouchings of the bladder wall).
Why Would Your Doctor Order a Bladder Ultrasound?
Your physician may recommend a bladder ultrasound to evaluate:
- Difficulty urinating or a weak urine stream — common in men with enlarged prostate
- Incomplete bladder emptying — feeling like you can’t fully empty your bladder
- Frequent urinary tract infections (UTIs) — retained urine can predispose to infection
- Blood in the urine (hematuria) — to check for bladder stones, masses, or other abnormalities
- Urinary incontinence or urgency — evaluating bladder capacity and function
- Pelvic pain or lower abdominal discomfort of unknown cause
- Suspected bladder stones or tumors
- Post-surgical evaluation — after prostate surgery, bladder surgery, or other pelvic procedures
- Monitoring urinary retention — in patients with neurological conditions that affect bladder function
- Pre-operative assessment for planned urological procedures
How to Prepare and What to Expect
You will typically be asked to arrive with a full bladder — drinking 32 ounces of water about one hour before your appointment is a common instruction. A full bladder provides better images and allows the sonographer to measure bladder volume and assess the bladder wall.
The sonographer applies gel to your lower abdomen and moves a transducer over the area. Images of the full bladder are captured. You’ll then be asked to empty your bladder, after which the sonographer scans again to measure how much urine remains (PVR).
The exam is painless and typically takes 15 to 20 minutes. There is no recovery time. A board-certified radiologist will review the images and send a report to your doctor.
