While ultrasound is an excellent tool for many abdominal conditions, its ability to directly detect stomach ulcers is limited. Understanding what ultrasound can and cannot show is important for getting the right diagnosis and treatment.

Can Ultrasound Find Stomach Ulcers?
In short: standard abdominal ultrasound is not the primary test for diagnosing stomach (peptic) ulcers. Ulcers are erosions in the lining of the stomach or duodenum (the first part of the small intestine), and they are best diagnosed through an upper endoscopy (esophagogastroduodenoscopy, or EGD). During an endoscopy, a doctor inserts a flexible camera through the mouth and directly visualizes the stomach lining, identifies ulcers, and can take tissue samples (biopsies) to test for Helicobacter pylori (H. pylori) bacteria, which is a common cause of ulcers.
Ultrasound’s limitations in detecting stomach ulcers include the fact that the stomach is a hollow, gas-filled organ, and air interferes with ultrasound waves, making the stomach lining difficult to image clearly. Ulcers are surface-level mucosal changes that are typically too small and subtle to be reliably detected by external ultrasound. The stomach’s position and variable distension further complicate imaging.
However, ultrasound may occasionally reveal indirect signs of a stomach problem, such as thickening of the stomach wall (which can be seen with advanced ulcers, tumors, or severe gastritis), fluid collections near the stomach, or signs of complications like perforation (free fluid or air in the abdomen).
When Is an Abdominal Ultrasound Helpful for Digestive Symptoms?
Even though ultrasound can’t reliably find stomach ulcers, it can be extremely valuable in the workup of upper abdominal symptoms because many conditions mimic ulcer pain. Your doctor may order an abdominal ultrasound to evaluate:
- Gallstones and gallbladder disease — one of the most common causes of upper abdominal pain, often confused with ulcer symptoms. Ultrasound is the gold standard for detecting gallstones
- Liver conditions — fatty liver disease, hepatitis, and liver masses can cause right upper abdominal discomfort
- Pancreatic abnormalities — pancreatitis and pancreatic masses can produce epigastric (upper middle) abdominal pain
- Kidney conditions — kidney stones or hydronephrosis on the right side can be confused with gallbladder pain
- Aortic aneurysm — in older patients, abdominal aortic aneurysm can cause deep abdominal or back pain
- General abdominal screening — providing a broad overview of the abdominal organs when the cause of symptoms is unclear
If your ultrasound is normal but your symptoms persist or are suggestive of an ulcer, your doctor will likely recommend an upper endoscopy for a definitive diagnosis. Blood tests and stool tests for H. pylori may also be ordered.
