CT Scan for Emphysema and COPD: How Imaging Helps Assess Lung Health

If you’ve been diagnosed with COPD (chronic obstructive pulmonary disease) or your doctor suspects emphysema, a CT scan of the chest can provide a far more detailed picture of your lung health than a standard chest X-ray. CT imaging reveals the extent and distribution of lung damage, helps distinguish emphysema from other lung conditions, and can guide treatment decisions — including whether you might be a candidate for advanced therapies.

How Does CT Imaging Help with Emphysema?

Emphysema is a form of COPD in which the tiny air sacs in the lungs (alveoli) are progressively destroyed, leading to difficulty breathing and reduced oxygen exchange. A high-resolution CT (HRCT) scan provides exceptional detail of the lung architecture and can reveal:

Areas of lung destruction. Emphysema appears on CT as areas of abnormally low density in the lungs — essentially “holes” where healthy air sacs have been destroyed. The pattern and distribution of these areas help classify the type of emphysema (centrilobular, panlobular, or paraseptal).

Severity and extent. CT can quantify how much of the lung is affected, using density measurements to calculate an “emphysema score.” This objective measurement is far more precise than what a chest X-ray or pulmonary function test can provide.

Bullae. Large, air-filled spaces (bullae) that form when adjacent alveoli merge after destruction can be identified and measured. Large bullae may compress healthy lung tissue and can sometimes be treated surgically.

Associated conditions. CT can detect bronchial wall thickening (chronic bronchitis component of COPD), bronchiectasis (damaged airways), pulmonary nodules, and lung cancer — which occurs at a higher rate in patients with COPD and emphysema.

Treatment planning. CT is essential for evaluating patients for lung volume reduction surgery (LVRS) or bronchoscopic lung volume reduction, as it shows exactly which areas of the lung are most damaged.


Who Should Get a CT Scan for Emphysema?

Your doctor may recommend a chest CT if you:

  • Have COPD and need a more detailed assessment of lung damage than pulmonary function tests alone can provide
  • Are being evaluated for lung volume reduction surgery or other advanced treatments
  • Have worsening symptoms that aren’t explained by your current diagnosis
  • Need screening for lung cancer (low-dose CT screening is recommended for high-risk patients with a significant smoking history)
  • Have unexplained shortness of breath and your doctor wants to rule out emphysema
  • Have alpha-1 antitrypsin deficiency, a genetic condition that causes early-onset emphysema

What to Expect During the Scan

No special preparation is usually needed for a chest CT. You’ll lie on a table that slides through the scanner and may be asked to hold your breath for 10 to 15 seconds during imaging. The scan itself takes about 10 to 15 minutes. Contrast dye is typically not needed for emphysema evaluation.

After the scan, you can resume normal activities immediately. A board-certified radiologist will interpret the images and provide a detailed report to your pulmonologist or referring doctor.
