How Are CT Scans Used for Pancreatic Cancer?
A specialized CT protocol called a pancreatic protocol CT is optimized for evaluating the pancreas. This technique captures images at multiple time points after IV contrast injection (called multiphase imaging) to show how the tumor enhances relative to normal pancreatic tissue. The contrast phases — arterial, portal venous, and delayed — each reveal different information.
Doctors use pancreatic protocol CT to detect the size and location of pancreatic tumors, determine whether the tumor involves critical blood vessels (the superior mesenteric artery, celiac artery, portal vein, and superior mesenteric vein) — this is essential for determining surgical resectability, identify whether cancer has spread to lymph nodes, the liver, or the peritoneum (lining of the abdomen), guide CT-guided needle biopsies when a tissue sample is needed for pathological confirmation, and monitor the cancer’s response to chemotherapy or other treatments.
Pancreatic cancer can often be seen as a mass on CT, appearing as an area that enhances differently from the surrounding normal tissue. When contrast dye is used, the mass typically stands out from healthy pancreas, making detection more reliable.
How Accurate Are CT Scans for Pancreatic Cancer?
CT is the primary imaging modality for pancreatic cancer evaluation and has a high detection rate for tumors, particularly those larger than 2 centimeters. The combination of CT with other tests such as endoscopic ultrasound (EUS), MRI, and blood markers (like CA 19-9) provides the most comprehensive evaluation.
CT’s main strength is its ability to show the relationship of the tumor to surrounding blood vessels, which is the most critical factor in determining whether the cancer can be surgically removed (resected). This vascular assessment is one of the most important contributions CT makes to pancreatic cancer care.
What to Expect During the Scan
If contrast dye is used (which is standard for pancreatic evaluation), you’ll be asked to fast for about 3 hours beforehand. You may also receive oral contrast to drink. At IPMC, you’ll change into a gown, lie on a cushioned table, and have an IV placed for contrast injection.
The scanner rotates around you, capturing images at multiple phases. The scan is painless. You may feel a brief warm sensation or metallic taste from the contrast — this is normal. The actual scanning takes about 10 to 15 minutes, with the full appointment lasting 30 to 60 minutes.
After Your CT Scan
You can resume normal activities immediately. Drink extra water if contrast was used. A board-certified radiologist will interpret the images and send a detailed report to your gastroenterologist, oncologist, or surgeon. Results are typically available within 24 to 48 hours.
