CT Scan for Bladder Cancer: Accuracy, Procedure, and What to Expect

How Are CT Scans Used for Bladder Cancer?

A CT scan of the abdomen and pelvis creates detailed cross-sectional images of the bladder, kidneys, ureters (the tubes connecting the kidneys to the bladder), and surrounding tissues. When optimized for the urinary tract (CT urography), images are captured at specific time points after IV contrast injection to show how contrast travels through the kidneys and collects in the bladder.

Doctors use CT scans for bladder cancer to detect tumors and masses within the bladder wall, identify whether cancer has spread to nearby lymph nodes, organs, or the ureters, stage the cancer to determine the best treatment approach (surgery, chemotherapy, immunotherapy, or radiation), evaluate the upper urinary tract for additional tumors (which can occur simultaneously), monitor how the cancer is responding to treatment over time, and check for cancer recurrence after treatment.


How Accurate Are CT Scans for Bladder Cancer?

CT scans are highly effective at detecting bladder and urinary tract cancers. Studies have reported accuracy rates of 96 to 97 percent for detecting upper tract urothelial cancers. For bladder tumors specifically, CT can identify most tumors, particularly those that are larger or have grown into the deeper layers of the bladder wall.

However, like all tests, CT has limitations. Very small or flat (sessile) tumors may be harder to detect, and the test may occasionally produce false positives or false negatives. This is why CT results are typically combined with other diagnostic methods — such as cystoscopy (a procedure where a camera is inserted into the bladder to look directly at the lining) and urine cytology — for the most accurate assessment.


What to Expect During the Scan

You may be asked to arrive with a full bladder, as a distended bladder is easier to evaluate on CT. Contrast dye is typically injected through an IV to help highlight the urinary tract structures. You’ll lie on a table that slides through the scanner, and multiple sets of images are captured at different time points as the contrast passes through your urinary system.

The scan usually takes about 20 to 30 minutes. The procedure is painless, and you may feel brief warmth or a metallic taste from the contrast injection. A technologist will be nearby to guide you throughout.


After Your CT Scan

You can go home and resume normal activities immediately. Drink extra water to help flush the contrast dye. A board-certified radiologist will review the images and provide a detailed report to your urologist or referring physician. Results are typically available within 24 to 48 hours.
